PLEASE NOTE: Once you book your appointment please join our website and complete the required forms and upload a picture or your valid Arkansas Drivers Licenses, or valid State of Arkansas issued ID, and a copy of your medical records confirming your qualifying condition diagnosis, (If we are not evaluating you for PTSD.)
